Mission: To improve the quality of life for our residents, patients, and their families during life’s health transition through compassionate, individualized care.
Created in 1989, Care Initiatives is the largest single operator of quality skilled nursing locations in Iowa. Today, Care Initiatives operates a total of 43 skilled nursing care locations, two assisted living locations, seven independent living locations, and eight hospice office locations. In 2024, Care Initiatives ventured outside the state of Iowa and opened Care Initiatives Hospice in Omaha, Nebraska.

Care Initiatives Hospice is committed to providing a high quality of life to patients and their families during their hospice journey. We provide coordinated care with compassion, respect and dignity to patients and their families experiencing serious illness and grief for a loved one.
Our team of compassionate health care professionals works together to serve patients and families. From personal physicians to counselors and volunteers, we’re committed to providing custom care for enhanced quality of life.
